You are hereby notified that this is a 30 day notice to you. As tenant(s) who reside in my rental
building, commonly known as ________________________. I have determined that you (and your
family members), are in serious violation of tenant Rental agreements through the following acts and
omissions, to wit:

If said breaches of the rental agreement and remedied/cured within 30 days of services of the Notice,
said rental agreement will be considered null and void and the tenancy terminates on the 31st day of
service of this Notice.
