As part of the Council agenda, a period for public comments shall be offered at the commencement of each regular Council meeting. Public comments will be noted in the City Council Minutes and become part of the official record. Those wishing to speak should sign their on addressed be to topic non-agenda item a agenda the and name or designated participation sheet. If there are five or fewer speakers, fifteen minutes shall be provided for Public Comment. If there are more than five speakers, a period of forty-five minutes shall be provided for all comment, and no individual shall speak longer than three minutes. The Mayor will allocate time among the speakers to ensure that Public Comment does not exceed forty-five minutes. The business of the City Council shall commence forty-five minutes after the beginning of Public Comment. Aldermen do not respond during Public Comment. (3800 east 91st Street, Cleveland, OH 44105) for Additional Civic Center Gutter and Downspout System Repairs in the amount of $100,341.50. Funding will be provided from the 2021 Capital Improvement Fund General Obligation Bond line item for Facilities Contingency (Account 415.40.4122.65515 – 621016) . This item has an FY 2022 budget of $600,000 of which $536,669.35 is remaining. For Action Approval of a Sole-Source Contract with Garland/DBS for Additional Civic Center Gutter and Downspout System Repairs - Attachment - Pdf 84 - 94 A4.Approval of a Sole Source Contract with Health Endeavors, S.C for Fire Department Annual Physical Fitness Testing and Medical Examinations Staff recommends City Council authorize the City Manager to execute the sole source contract with Health Endeavors, S.C. in the amount of $41,511 health and testing fitness provide annual the physical to screening required for union employees in the Fire Department. The Fire Department will pay for this out of the General Fund, account 100.23.2305.62270, which has an approved budget of $50,000 in Fiscal Year 2022, with $49,198.96 remaining YTD. For Action Approval of a Sole Source Contract with Health Endeavors, S.C for Fire Department Annual Physical Fitness Testing and Medical Examinations - Attachment - Pdf 95 - 100 A5.Approval LS Contract with to 1 No. Order Change of the Contracting Group, Inc. for Parking Garage Structural Repairs (Bid 22-14) Staff recommends approval of Change Order No. 1 to the contract with LS Contracting Group, Inc. This will increase the overall contract price from $1,002,947.00 to $1,028,547. There is no time extension associated with this change order. Funding for this change order will initially be provided from the Parking Fund (Account 505.19.7005.65515-620010), in the amount of $25,600. This amount will be reimbursed by the Chicago Avenue Place Condo Association following completion of the work. This will increase the overall contract amount from $367,249.30 to $398,274.71.
