A RESOLUTION
An Employer Participation Agreement
with PEBSCO for a Post Employment Health Plan
for Certain Collectively Bargained Public Employees
WHEREAS, the City of Evanston is a State or a political subdivision thereof; and
WHEREAS, the Post Employment Health Plan ("Plan") provides post -retirement
reimbursement of qualifying medical care expenses for the benefit of government
employees and their dependents; and
WHEREAS, pursuant to a ratified and approved 1999-2001 collective bargaining
agreement with the Evanston Fire Fighters Association, Local 742 of the International
• Association of Fire Fighters (the `Union"), the City has agreed to make contributions
pursuant to the Plan for work performed by its employees covered by said collective
bargaining agreement ("contributions"); and
WHEREAS, the Contributions will be held in trust by the LaSalle National Bank, or
its successor, as trustee (the "Trustee") of the Trust for the Post Employment Health Plan
for Collectively Bargained Public Employees (the "Trust') for the exclusive benefit of Plan
participants and their qualified dependents; and
WHEREAS, the Plan gives authority to the Administrator to accept on behalf of the
Plan an Employer for participation in the Plan; and
WHEREAS, the Administrator is willing to accept the City as an Employer under the
• Plan upon the terms and conditions herein set forth; and

WHEREAS, the City Council has approved the current collective bargaining
agreement including participation- in said Plan,
NOW THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF
EVANSTON, COOK COUNTY, ILLINOIS:
SECTION 1: That the City Manager is hereby authorized and directed to sign,
and the City Clerk hereby authorized and directed to attest on behalf of the City of
Evanston, an Employer Participation Agreement by and between the City of Evanston and
Public Employees Benefit Services Corporation ("PEBSCO") for a Post Employment Health
Plan for Collectively Bargained Public Employees. Such Agreement shall be in substantial
conformity with the Agreement marked as Exhibit A attached hereto and incorporated
herein by reference. •
SECTION 2: That the City Manager is hereby authorized and directed to
negotiate any additional conditions of the Agreement as may be determined to be in the
best interests of the City.
SECTION 3 That this Resolution shall be in full force and effect from and
after its passage and approval in the manner proVided by law.
