HomeMy WebLinkAboutRESOLUTIONS-2015-090-R-158/24/15 90-R-15 A RESOLUTION Authorizing the City Manager to Sign Notifications of Grant Awards To Fund and Operate a Congregate Senior Meal Program WHEREAS, the City of Evanston has made it a priority to focus on providing for a comfortable and high stand of living for seniors through activities and events; and WHEREAS, the Parks, Recreation and Community Services Department (the "Department") strives to provide activities and opportunities to seniors; and WHEREAS, the Department applied to the Illinois Department on Aging for funding for a congregate senior meal program; and WHEREAS, the Department received two (2) grant awards, worth a combined forty-five thousand, two hundred sixty-four dollars ($45,264), to operate meal programs for seniors; and WHEREAS, the grant award will allow seniors to receive meals at a suggested donation rate of four dollars and fifty cents ($4.50); and WHEREAS, the Department plans to operate a lunch program for seniors five (5) days a week at the Levy Senior Center; and WHEREAS, the Department plans to operate a lunch program for seniors ones (1) day a week at the Fleetwood-Jourdain Community Center, NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF EVANSTON, COOK COUNTY, ILLINOIS: 90-R-15 SECTION 1: That the foregoing recitals are hereby found as fact and incorporated herein by reference. SECTION 2: That the City Manager is hereby authorized to sign, and the City Clerk herby authorized to attest, on behalf of the City of Evanston, the Notifications of Grant Awards. SECTION 3: That the City Manager is hereby authorized to sign, and the City Clerk herby authorized to attest, on behalf of the City of Evanston, the Notifications of Grant Awards attached hereto as Exhibit A, incorporated herein by reference. SECTION 4: That this Resolution 90-R-15 shall be in full force and effect from and after its passage and approval in the manner provided by law. I am pleased to notify you that your agency was approved for funding as described below. The award to your agency is for the following amounts for the described service(s) in the specified service areas: CI Congregate Meals.- Citv of Evanston Service Award Level Units Congregate Meals- Levy Senior Center $46,866 7,158 Congregate Meals- Fleetwood Jordain Center $12,795 1,560 Total Title III-C1 Award $59,661 8,718 Note that the funding level described above is pending final caterer rate/costs that result from the Unified Caterer Bid process. If the caterer meal rate, as selected from the Unified bid, is: a. Higher than the estimate, AgeOptions will cover the higher cost; or b. Lower than the estimate, AgeOptions will reduce the award by the difference of actual and estimate. AgeOptions anticipates that reimbursement for the state share of funding will continue to be delayed in FY 2016. Budget pages: Title Ill-C budget pages that reflect the FY 2016 allocation, as outlined in this letter. Please note that a separate budget must be submitted for each Title III-C program funded (e.g., one budget for each III-Cl site; one for each III-C2 distribution point). b. FY 2016 Reimbursement Request by month for each budget submitted. 2. The Illinois Department on Aging must approve the AgeOptions FY 2016 Amendment to the Fiscal Years 2016-18 Area Plan on Aging. 3. AgeOptions must receive sufficient funds from the Illinois Department on Aging to meet the total obligations for the period October 1, 2015 to September 30, 2016. 4. There must not be any administrative, legislative order, judicial order, rule or law which requires a change in this or a related decision made by AgeOptions. If all documentation and contingencies identified in item 1 are not complied with by August 21, 2015, the letter of intent shall be null and void as of August 21., 2015. August 21, 2015 is a finn deadline and cannot be extended; any documentation received after the close of business on August 21, 2015 will not be considered timely. A copy of the AgeOptions Appeals Procedure is attached to this letter and should be considered part of this award announcement. Please note that upon receipt of a notice of appeal, AgeOptions shall attempt to informally resolve the dispute that is the subject of the appeal within ten calendar days of receipt of the appeal. If your agency's decision to appeal is unchanged, the next step is to submit a request within 15 calendar days of the informal resolution discussion/communication. This Letter of Intent is not a binding contract. The parties' obligations are subject to execution of a final agreement.
